Initial Comments:
Based on the findings of an off-site follow-up survey conducted on June 14, 2022, Bridge Care Hospice, Inc., has corrected the deficiencies cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a state re-licensure survey completed March 22, 2022.
